Home & Community Habilitation

This direct service consists of services tailored to helping individuals acquire, retain, and improve self-help, socialization, and adaptive skills.

Respite Care – Home Based

Wherever our individuals call their home, our team will be there to take over the caregiving duties of their primary caregivers.

Companion Service

We can send our staff to keep our individuals company.

Homemaker/Chore Services

We have a team of dependable staff who can do various household chores.

Job Support

We partner with employers and community resources to help our individuals become employable.

Registered Nurse and Licensed Practical Nurse

We offer nursing services to support our individuals’ medical needs.

Job Finding

Our team coordinates with a vast network of employers where individuals we serve can land a job that they want and/or that’s right for them.

Community Integration (Also called Community Participation Support (CPS)

We provide all the support we can to help individuals with intellectual disabilities participate in the community.
